Hi Peter,
That is a great reference, thanks. Indeed, the OIG and CMS have been relatively silent on the topic but their actions, or lack of therein, has spoken louder than words. Not once have we heard of successful litigation regarding a hospital or physician undercharging self pays. as long as there is a well documented policy it is acceptable to charge self-pays a reasonable fee or forgive deductibles - but there is a catch. It can not be for the goal of attracting more medicare business, so it can not be marketed as such. This is why there is some modesty built into the FairCareMD price transparency system. List a rate above the Medicare rate and if a patient requests it, you can possibly go below if the patient fits the eligibility criteria.

Additionally, the prompt pay discounts enabled in most states allow medical facilities and providers to allow patients to pay significantly less than insured or delayed payment care.

I am sure you reviewed the info on our legal tab.

Referral Click is pretty neat too - Provider to Provider referral tool. FairCareMDs use our site for this too sometimes but mostly they just tell the patients to go to FairCareMD, leaving the patient in charge. It is easier that way, especially these days when referrals are so complicated and FairCareMD patients tend to be active healthcare consumers rather than passive ones.

Great post - some clients expressed concern over the "less than contracted rates" question when offering these cash pay discounts.. This post provides great ammo..

Have you received any guidance from CMS on the subject - I can only find a document from 2004 on the self-pay issue..??

http://www.cms.gov/AcuteInpatientPPS/downloads/FAQ_Uninsured.pdf

Thx!

Thanks Dr. Engelmayer!
The support and appreciation of clinicians of all types (MD, PhD, PsyD, DC, DOs, PA, RN, DDS, DMD, and many more) is what drives this site and my motivation to keep it going. We always need more docs in the system for our patients (who outnumber our docs by over 100:1, so please be welcome to set up your account as a cancer care consultant, or other more appropriate term. This is actually a common request we get that we have trouble filling due to the complexity. It really takes a specialist such as yourself. In fact, if you are interested, I have three patients I could refer you too now. The most efficient thing, of course, is that you and the providers get listed on the site. The whole idea of FairCareMD is to automate the process of learning and finding exactly what you need and arranging a fee, all online - like an ATM machine vs. a financial planner. Add an HSA to the mix and people stand to save well over half their medical costs!

Since we are free for most oncologists, I am sure we will be happy to have your favorite physicians. Here is a great example: Dr. David Fishman, Gynecological Oncology and head of the National Ovarian Cancer Early Detection program (and a heck of a nice guy.)

Once again, thank you for your appreciation - something, like doctors in the system, we can never get enough of!

Be Well,
Alex

Congratulations of your site Alex. I think your service is a great help for those uninsured and under-insure. It goes a long way of improving communication between doctors and patients, so that the latter can better understand and evaluate the condition and physician before a consultation.

Sincerely,
Jose Engelmayer, PhD

Hey Mike,
Sounds like you need to get your profile set up on the main site. We bring great SEO to your practice website too! It only takes about 30 minutes and gets you patients willing to pay a fair price for your services. Try it, the patients are waiting!
Good Luck,
Alex

I really enjoyed this post. You write about this topic very well. In bringing Medical House Call services to patients, the hands of time are essentially turning back. I remember how medicine used to be practiced, before busy doctor's offices, complicated insurance policies, long lines at clinics, rushed office visits, and crowded emergency rooms. I've noticed that somewhere along these channels of annoyance, doctors began to lose sight of what mattered most- their patients. I remember when the doctor and patient were able to develop trustworthy relationships through Medical House Calls and had all the time necessary to discuss, examine, diagnose, and treat their condition. The idea behind developing this type of practice model was to minimize the understandable aggravation that both patients and physicians have grown accustomed to in busy todays medical practices. Medical House Calls makes a doctor visit worry free and relaxed in your own surroundings.
